Career After MBBS in NE India: PG, Government Service and Practice Pathways
An MBBS degree from an NE India college opens pathways to NEET-PG, government service, central institution careers and private practice. Here is how to plan the journey from Day 1 of MBBS.

NEET-PG: The Primary Goal for Most Graduates
NEET-PG is the gateway to MD and MS specialisations. It is conducted by the National Board of Examinations (NBE) once a year, though the exact month has shifted around considerably in recent years (NEET-PG 2026 is scheduled for 30 August 2026) — always confirm the current year's date from the official NBE notification rather than assuming a fixed month. It tests the full breadth of clinical and pre-clinical MBBS subjects. Approximately 2.5 lakh candidates appear annually for roughly 50,000–65,000 combined MD/MS/PG-Diploma and DNB seats. Preparing for NEET-PG effectively requires structured study from the third year of MBBS — not just during internship.
PG Seats Available in NE India
NE India has significantly expanded PG capacity. NEIGRIHMS Shillong (premier central institution) offers MD, MS and super-speciality seats across nearly 30 departments. GMCH Guwahati and RIMS Imphal run MD/MS programs in major clinical departments. AIIMS Guwahati, which became fully operational for clinical programs by 2024, adds further PG and super-speciality capacity. Completing MBBS from NE India and applying for PG in the same region is now a fully viable and increasingly competitive pathway.
Government Service and Bond Completion
Most NE state government MBBS graduates must complete a service bond of 2–3 years before or concurrent with PG preparation. Government service as a Medical Officer at a Community Health Centre or District Hospital provides a monthly salary of ₹65,000 to ₹90,000 (2026 pay scales, most NE states) plus allowances. This income allows you to fund NEET-PG coaching while fulfilling your bond. Plan to appear for NEET-PG in your second or third year of bond service.
Central Institutions in NE India
AIIMS Guwahati
AIIMS Guwahati was established under the Pradhan Mantri Swasthya Suraksha Yojana. It now runs MBBS, MD/MS and fellowship programs. For NE MBBS graduates, a PG seat at AIIMS Guwahati carries AIIMS brand recognition nationwide. Admission to MD/MS at AIIMS Guwahati is through NEET-PG; some specialisations are filled via INI-CET.
NEIGRIHMS Shillong
NEIGRIHMS is a premier autonomous body under the Ministry of Health offering MD, MS, DM and MCh programs. PG admissions are through INI-CET. The institute has a strong focus on NE-specific public health and disease patterns, making it especially relevant for doctors planning to practise in the region.
Private Practice and Entrepreneurship
NE India has a significant shortage of specialists and general physicians in semi-urban and rural areas. After PG (or even after bond service as a general practitioner), private practice in NE India can be highly rewarding — both financially and in terms of impact. The doctor-to-population ratio in Nagaland, Mizoram and Arunachal Pradesh is far below national averages, meaning lower competition and higher demand. Several MBBS graduates who returned to their home districts after bond service built successful practices within 3 to 5 years.
Planning Your MBBS-to-PG Journey
Years 1–2 (MBBS): Build strong foundations in Anatomy, Physiology and Biochemistry — these are heavily tested in NEET-PG
Year 3 (MBBS): Begin structured NEET-PG subject-wise reading alongside clinical studies
Year 4 (MBBS) + Internship: Intensify NEET-PG preparation; attempt at least three full mock tests per month
Bond Year 1: Appear for NEET-PG in March; use on-call duty gaps for revision
Bond Years 2–3: If NEET-PG rank achieved, check your state bond waiver policy for in-service PG admission
Post-PG: Explore super-speciality (DM/MCh via NEET-SS) or fellowship at AIIMS, PGI or NEIGRIHMS
NE doctors in high demand
Central government schemes like the National Health Mission (NHM) specifically incentivise doctors to serve in NE India with additional rural allowances. Several NE states offer fast-track promotions in the state health service for doctors who complete their bond and continue in government service. The career floor in NE Indian government medicine is stable and growing.
