NEET PG Counselling: AIQ, INI-CET and State Quota Explained

Three Separate Pathways, Not One
Clearing NEET PG qualifies you for two of the three routes to a government MD/MS seat — AIQ PG and state PG quota. It does not qualify you for INI-CET, which is a completely separate entrance exam for Institutes of National Importance and does not use your NEET PG score at all. Confusing the two is one of the most common planning mistakes MBBS graduates make.

A common and costly mistake
MBBS graduates sometimes assume a strong NEET PG score alone opens the door to NEIGRIHMS or AIIMS MD/MS seats. It does not — those institutes fill their PG seats through INI-CET, a separate exam with its own registration, syllabus emphasis and exam date. If NEIGRIHMS or an AIIMS campus is on your list, you need to prepare for and register for INI-CET independently of NEET PG.
What This Means for NE India MBBS Graduates
Regional PG capacity has grown meaningfully in recent years. RIMS Imphal runs a large MD/MS programme across 16+ specialties, filled entirely through MCC AIQ PG counselling. GMCH Guwahati also offers MD/MS through AIQ PG across major clinical departments. AGMC Agartala offers MD/MS in 8+ specialties through a mix of AIQ PG and state PG quota. NEIGRIHMS Shillong — a premier central institution — offers MD, MS and super-speciality seats across nearly 30 departments, but exclusively through INI-CET, not MCC or any state DME process.
How AIQ PG and State PG Quota Counselling Work
The process broadly mirrors NEET UG counselling: online registration on the MCC portal (mcc.nic.in) for AIQ PG, or your state DME's portal for state PG quota, followed by choice-filling and multiple allotment rounds. Registration fees and security deposits apply and are revised most years — always confirm the current amounts from the official information bulletin for that cycle rather than assuming a previous year's figure still applies. You can register for both AIQ PG and your home state's PG quota simultaneously to maximise your options, exactly as with NEET UG.
Specialty Selection: A Regional Angle Worth Considering
Beyond personal interest and aptitude, regional healthcare demand is a genuine factor for MBBS graduates planning to practise in North-East India. Specialties with consistently strong demand at NE state hospitals include General Medicine, Paediatrics, Anaesthesiology, Obstetrics & Gynaecology, Orthopaedics, Radiology (Radio-diagnosis) and Psychiatry — these also tend to represent strong government-service opportunities in the region. This is contextual planning information, not a prediction of seat availability or a suggestion to choose a specialty you are not genuinely interested in.
Service Bonds on State PG Quota Seats
Many state PG quota seats carry a service bond, similar in principle to the bonds attached to state-quota MBBS seats — but the exact amount, duration and terms vary by state and sometimes by specialty, and can change from year to year. Confirm the specific bond terms for the college and specialty you are targeting directly with that state's DME before you commit; do not assume UG-level bond figures carry over to PG.

What is the difference between AIQ PG, state PG quota and INI-CET?
