Domicile requirements for Meghalaya state quota
For Shillong Medical College's Meghalaya state quota, domicile is established through the Inner Line Permit (ILP) system (for certain areas), birth certificate showing Meghalaya birth, school certificates from Meghalaya institutions, and parents' employment in Meghalaya state government or residence proof — verify the exact current-year list with DMER Meghalaya, since this is a newly operational counselling process.
For NEIGRIHMS's own Meghalaya-linked state-quota seats, domicile requirements are set by NEIGRIHMS itself as part of its official (non-MCC) admission process, not by DMER Meghalaya. For NEIGRIHMS's AIQ seats, domicile of Meghalaya is not a requirement — that share is open to all through MCC.
Overview
Meghalaya's medical education landscape now has two government MBBS options in Shillong. NEIGRIHMS — the North Eastern Indira Gandhi Regional Institute of Health and Medical Sciences — is a premier central government institution (50 MBBS seats: 8 AIQ via MCC, 14 reserved for Meghalaya, 10 for Nagaland, and 18 as an "NE Open" category for any of the 8 NE states), with its state-linked seats filled through NEIGRIHMS's own official admission process, not through Meghalaya's state DME. Since AY 2025-26, Meghalaya also has its first true state government medical college — Shillong Medical College — with 50 MBBS seats (42 Meghalaya state quota, 8 AIQ), allotted through MCC AIQ and Meghalaya DME state quota counselling.
Meghalaya domicile students seeking a state-quota MBBS seat now have a genuine home-state route through Shillong Medical College, in addition to NEIGRIHMS's own separate state-linked allocation and the national AIQ pool. Students from Meghalaya also continue to access MBBS through AIQ at government colleges across India, or Management Quota at private colleges.
Always verify both institutions' current-year seat matrix and NMC recognition status on the official NMC website and the DMER Meghalaya portal before making admission decisions, since Shillong Medical College is a very new institution.

Meghalaya students now have two Shillong-based government MBBS routes: Shillong Medical College's state quota (42 seats, competing only against other Meghalaya domicile students) and NEIGRIHMS's Meghalaya-linked allocation within its own admission process. Both let students study in their home state.
For Meghalaya students not securing either route: AIQ gives access to government college seats across India, and Management Quota in Karnataka, Telangana or other open states is the primary private route. Students from Meghalaya also have access to GMCH Guwahati and AMC Dibrugarh AIQ seats.
